在天地之间，数千年的沧海桑田孕育了无数神秘的族群。妖魔为了满足自身贪欲，侵蚀人类世界，而在这动荡的环境中，部分人类诞生了拥有特殊能力的存在。其中，画皮师以其神奇的画笔能力著称，能够化腐朽为神奇，重塑人的容貌和身份。画皮师自古以来都传承着一师一徒的古老秘技。
故事的核心围绕着最后一代画皮师方祭（蔡俊涛 饰）及其徒弟穆一骞（芮伟航 饰）。他们游历四方，行医救人，却在龙湖镇卷入了一宗扒皮灭门案件。穆一骞因误会被抓入衙门，方祭则在调查过程中救下了唯一的幸存者珊瑚（尹玲 饰），并遇到了同样调查此案的捉妖师梁佑言（赵威霖 饰）。随着案件的深入，方祭发现这一系列事件竟然与三十年前的一段秘密往事息息相关。原来，背后的凶手竟是一个依靠食人脸皮维持容貌的妖。这一发现引发了一场人与妖之间的正邪大战，也揭示了隐藏在案件背后的更深层次的阴谋和秘密。
